Check the following:

• Brake is lifting correctly. (Check Brake and Brake Circuit).
• Lubricate car/counterweight guides.
• Check for stiction in car/counterweight guides.
• Check motor encoder wiring.
• Possible faulty motor encoder.
• Excessive Accel/Decel rates.
• Check motor encoder coupling.
